If you are from UK log into your account and do the steps without use any proxy.
If you are from anywhere else you will need to use a web proxy, go to justproxy.co.uk or quickproxy.co.uk
After setting the proxy go here: www.stardoll.com
Log into your account and now paste this into the proxy URL:
http://www.stardoll.com/en/clubs/home.php?id=4826173&action=ma&ac=4826173
Joining the club you will receive the guitar.
Now, to receive the totem bag, just scroll down the page until you find this image; click on the bag image and you will be redirected to facebook page, wait until the page loads and done, you can close that window.
Done!! Now you can leave the proxy and go to normal Stardoll.xoxo Mia :)
